Horizon Healthcare Introduces Weight Management Program - 2009-05-13

Horizon Healthcare, an affiliate of Fairfield Memorial Hospital, is now offering a complete program for a healthy lifestyle.  This program will include weight management individualized for the patient based on the patient’s personal needs and health conditions.  The program includes a complete health assessment by a medical provider, medication management, food frequency assessment, nutrition planning by a licensed nutritionist, and an individualized exercise plan.  Periodically, there will be free group educational meetings on topics of interest held at Fairfield Memorial Hospital. 

 

Lois Dishman, MPAS, PA-C, will be the provider who will be guiding patients through the weight management program.  Physician Assistant Lois Dishman is a family practice provider and has her masters in Physician Assistant Studies/Geriatrics and specializes in women’s health.  She will receive assistance with the nutritional planning for patients by the Fairfield Memorial Hospital’s Licensed Nutritionist, Brian Carroll MS, RD, LDN. 

 

Program costs will be $74 for the initial evaluation and additional charges apply for lab services, if needed.  Subsequent weight management visits are only $25. 

 

“We know losing and maintaining a target weight is a common struggle, but success in this area can lead to many improvements in an individual’s health and well-being.  With PA Dishman’s commitment to helping people manage their weight, Horizon is offering an affordable option since this service is not covered by medical insurance,” explained Julie Aman, Clinic Director.
