Levels of Giving

Recognition on Donor Wall of Honor

All giving is cumulative, starting with the 1998 “Keep the Dream Alive” campaign. The Donor Wall will be updated periodically to reflect increased level of giving in each category. Donations can be made at any time to the Fairfield Memorial Hospital Foundation.

Name Plate Levels:

  • Pearl $500 - $1,499
  • Amethyst $1,500 - $4,999
  • Sapphire $5,000 - $9,999
  • Ruby $10,000 - $14,999
  • Emerald $15,000 - $19,999
  • Diamond $20,000 - $24,999

Suggestions for Name Plates:

  • Couples: Mr. & Mrs. Joe Jones
  • Honors: In Recognition of Joe Jones
  • Memorials: In Memory of Joe Jones
  • Businesses: ABC Corporation

Plaque Levels:

  • Distinguished Bronze $25,000 - $49,999
  • Distinguished Silver $50,000 - $74,999
  • Distinguished Gold $75,000 - $99,999
  • Distinguished Platinum $100,000 - $249,999
  • Distinguished Titanium $250,000 and up

Plaques can include an etched picture of the donor(s) or working at the donor’s request.
