EDUCATION/QUALIFICATIONS

· Required: Master’s degree in Social Work (MSW) from an accredited Graduate School of Social Work, or a related degree that supports licensure

· Preferred: LMSW, LCSW, LPC, or LMFT

· Preferred: 2–3 years of clinical experience working with older adults

· Preferred: Prior experience in a hospital or nursing home setting

· Preferred: Familiarity with Joint Commission, Medicare, and state licensure standards

EXPERIENCE

· Preferred: 2–3 years of clinical experience with older adults

· Hospital or nursing home experience strongly valued

· Prior experience in treatment planning, discharge coordination, and interdisciplinary collaboration

SKILLS

The ideal candidate will bring strong interviewing and assessment abilities, with the capacity to develop comprehensive psychosocial evaluations and treatment plans. They should be proficient in conducting individual, group, and family therapy sessions, demonstrating flexibility in adjusting treatment modalities to meet patient needs. Excellent documentation and organizational skills are essential, along with the ability to collaborate effectively with physicians, nurses, and interdisciplinary team members. Candidates must uphold patient rights by maintaining confidentiality, dignity, and respect in all interactions. Leadership skills are important for supervising interns and representing the program in educational settings. Above all, professionalism, accountability, integrity, and compassionate communication are expected in every aspect of performance.

NATURE OF WORK

The Senior Life Solutions Primary Therapist provides psychotherapy and social services to clients, focusing on treatment planning, discharge coordination, and collaboration with interdisciplinary teams. This role involves conducting assessments, leading therapy sessions (individual, group, and family), and ensuring compliance with hospital, state, and federal standards. The therapist also supervises social work interns and contributes to quality improvement initiatives.
