    Population Health Nurse Educator

    Minimum Requirements:

    EDUCATION/QUALIFICATIONS

    • Required: Professional Registered Nurse in State of Illinois.
    • Preferred: Bachelors of Science in Nursing

     

    EXPERIENCE

    • Required: Minimum of 3 years of clinical nursing experience.
    • Preferred: 5 years of experience in population health, chronic disease management, or care coordination.

     

    SKILLS

    • Strong clinical knowledge in chronic disease management (diabetes, hypertension, etc.).
    • Excellent communication and patient education skills.
    • Ability to collaborate with interdisciplinary teams and community partners.
    • Proficiency in electronic health records (EHR) and quality reporting systems.
    • Strong organizational and problem-solving skills.
    • Commitment to patient-centered care and continuous improvement.

    NATURE OF WORK
    The Population Health Nurse Educator is responsible for enhancing the quality of clinical outcomes, improving patient satisfaction, and managing the cost of care through education, care coordination, and community outreach. This role empowers patients to actively participate in their care plans, improves care transitions, and strengthens patient–provider communication. The Nurse Educator supervises education for patients, families, caregivers, and healthcare professionals, with emphasis on chronic disease management (diabetes, hypertension, and other conditions), social determinants of health, and transition of care management.

    This position also leads quality improvement initiatives, including monthly chart audits, electronic quality reporting for regulatory compliance, and performance improvement activities under the Merit-Based Incentive Payment System (MIPS).

     

    KEY RESPONSIBILITIES

    • Collaborate with practitioners, caregivers, and patients to coordinate care and eliminate barriers.
    • Communicate and document care plans to ensure continuity of care.
    • Provide patient and family education on chronic disease management (e.g., diabetes, hypertension).
    • Support discharge planning and care transitions.
    • Conduct quality chart audits and manage reporting for compliance and performance improvement.
    • Engage in community outreach and support groups to promote patient self-management.
    • Encourage patient portal use and connect patients with available resources.
    • Demonstrate professionalism, teamwork, and adherence to hospital policies.
