    SCU LPN – PRN
    Department: FMH Skilled Care Unit
    Minimum Requirements:

    EDUCATION/QUALIFICATIONS 

    • Required: Graduation from an accredited School of Nursing; Current LPN licensure in the State of Illinois; BLS certification. 
       

    EXPERIENCE 

    • Preferred: Minimum 1 year of CNA or LPN experience. 

     

    SKILLS 

    • Ability to read, write, speak, and understand English. 

    • Strong computer skills with the ability to navigate EMR systems. 

    • Ability to work independently, prioritize tasks, and manage time effectively. 

    • Excellent communication and customer‑service skills.

    NATURE OF WORK 

    The Skilled Care Unit LPN works under the supervision of a Registered Nurse to implement the nursing plan of care for residents. This role provides direct nursing care, supervises CNAs and supportive staff, ensures continuity of care between shifts, and actively participates in resident and family education. The LPN serves as a resident advocate and upholds the mission, ethics, and goals of the facility. 

     

    KEY RESPONSIBILITIES  

    • Provide direct nursing care and perform ongoing resident assessments, documenting changes in condition and care in the SCU EHR. 

    • Implement and update the Plan of Care in collaboration with the Interdisciplinary Care Team, ensuring resident and family involvement. 

    • Administer medications and treatments safely, including permitted IV therapy tasks, and follow all documentation standards. 

    • Communicate effectively with physicians, reporting changes in condition, lab and diagnostic results, and carrying out orders promptly. 

    • Ensure thorough shift handoffs, sharing updates on condition changes, new orders, pending results, and other essential information. 

    • Supervise and delegate to CNAs, supporting high-quality resident care and assisting with orientation of new staff. 

    • Maintain infection control practices, ensuring compliance among supportive staff. 

    • Serve as a resident advocate, protecting resident rights and providing education to residents and families. 

    • Respond calmly and effectively in emergencies, demonstrating sound judgment and professionalism. 

    • Support pain management practices, providing care that meets both psychosocial and physical needs. 

    • Collaborate with the healthcare team, contributing to problem-solving, continuity of care, and a positive unit environment. 
    • Adhere to hospital policies, professional standards, and regulatory requirements, maintaining confidentiality and demonstrating consistent professionalism.
