    Horizon Healthcare Achieves Target: BP Silver+ Recognition

    Horizon Healthcare is proud to announce it has been awarded the Target: BP Silver+ Achievement Level by the American Heart Association (AHA) and the American Medical Association (AMA). This national recognition honors Horizon Healthcare’s dedication to improving blood pressure control through evidence-based strategies and exceptional patient care.

    The Target: BP program is a collaborative initiative of the AHA and AMA that helps healthcare organizations and communities improve blood pressure control rates and reduce the number of Americans who suffer heart attacks and strokes.

    By reaching the Silver+ Achievement Level, Horizon Healthcare has demonstrated consistent excellence in:
    • Measuring Accurately – Implementing best practices for precise blood pressure measurement.
    • Acting Rapidly – Quickly identifying and managing patients with uncontrolled blood pressure.
    • Engaging in Additional Pillars – Taking proactive steps in another key area of the program to support long-term cardiovascular health.
