Job Shadowing

Welcome to Fairfield Memorial Hospital Job Shadowing Program. We encourage you to choose a health career field and are very happy to offer this opportunity.

The goal of job shadowing is to introduce you to the healthcare profession. If you are interested in healthcare as a career, we would like to give you the opportunity to job shadow at FMH.

Please complete the following steps:

1. Download, fill out and upload the Job Shadow Packet.
2. Compile health documents required. (If you are a current employee, your health documents are not required.)
3. Upload the following required health documents:
   1. Proof of immunization history, including MMR, TDAP, Hepatitis B and Varicella
   2. Proof of TB test with negative result given within the last 12 months – must be a two-step TB skin test, QuantiFERON gold, or a T-Spot lab test
   3. Proof of seasonal flu vaccination or declination (required for observations taking place between Sept. 1 and March 31)
