DAISY Award for Extraordinary Nurses
The DAISY (Diseases Attacking the Immune System) Award is an international recognition program that honors and celebrates the skillful, compassionate care that
nurses provide everyday. The DAISY Foundation was established by the family of J. Patrick Barnes after he died from complications of the auto-immune disease ITP in 1999. During his hospitalization, they deeply appreciated the care and compassion shown to Patrick and his entire family. When he died, they felt compelled to say “thank you” to nurses in a very public way. Deaconess is proud to be a DAISY Award partner, recognizing DAISY Award winners across multiple campuses throughout the year.

The DAISY Award is an international program that rewards and celebrates the extraordinary clinical skill and compassionate care given by nurses every day. Fairfield Memorial Hospital is proud to be a DAISY Award partner, recognizing our nurses with this special honor throughout the year.

Who can nominate a nurse for the DAISY Award?
Patients, families, or visitors may nominate a deserving nurse by filling out a nomination form and submitting it in one of the nomination boxes around the hospital or by completing the online form below.
When will the DAISY Award be given at Fairfield Memorial Hospital?
Combined awards: FMH will honor two DAISY award winners each year - one in July and one in December.
